Middle School Community Service Contest

Attention Middle School Crosby Scholars!!!

You could be a lucky winner of a $25 gift card just by completing your community service over the summer!

Volunteer this summer, and turn in your community service form by 12pm on August 16^th verifying at least two hours of community service, and your name will go into a drawing for a $25 gift card. We are giving away FIVE gift cards. You will be able to choose your card:  Target, Walmart, Visa, Sheetz, or Starbucks.

Community service forms for the 2019-20 school year are posted on our website, or click here to download a form.

Submitting your form is easy!  It must be turned in one of the following ways. You may not submit a form at your school for this contest.

1. Upload your form to your Crosby Scholars Student Portal on the My Documents page, which is the dropdown of the My Crosby page. You will see a field for the 2019-20 community service form.
2. Take a picture of your form and email it to middleschool@crosbyscholars.org
3. Mail it to Crosby Scholars Program, 2701 University Parkway, W-S, NC 27105 – take a picture of your form prior to mailing it
4. Fax it to 336.725.1321
5. Drop it off at the Crosby Scholars office
